The Hospital Victoria Eugenia de Sevilla Health Card has been created for the benefit of groups of people, so that they can receive health care at our facility with special rates and more personalised service, and to keep them fully abreast of the entire portfolio of services that may be of interest to them.
Registration
Interested parties may sign up to become Cardholders by filling out and signing the registration form:
- Individuals over 18 years of age.
- All children must have cards as beneficiaries, dependent upon a Cardholder
- Legal persons are not eligible
Participation in this program is governed by the usage rules and conditions in this contract, to which the holder provides his agreement and consent. These conditions may be modified or cancelled by the Hospital Victoria Eugenia de Sevilla at any time, with or without advance notice. A copy of these conditions will be available to the cardholder at www.hospitalveugenia.com.
In the event of the program’s termination, after notifying the Cardholders, all the advantages offered them under it may be rescinded.
In order to be eligible to become a cardholder one must correctly complete and sign the registration form, and read and accept these general conditions. The initial use of the card implies full acceptance of the general conditions.
An individual may only hold one card.

With the Hospital Victoria Eugenia healthcare card, consults in any specialisation cost only €37* .
Without having to pay a monthly fee, with this card one can enjoy special rates, which can be viewed at www.hospitalveugenia.com
As a holder of this card you can enjoy access to the services offered in the hospital’s Portfolio of Services at special rates.
Patients may only directly access Medical Consults. For everything else a physician’s prescription will be necessary.
All holders of this card may make appointments for Outpatient Care through the appointment programme on the website at www.hospitalveugenia.com
Cardholders who provide their email addresses will also have special access to information via this channel, and to a Customer Care Centre at 955 03 86 00 or 954 35 14 00.

Conditions
Only holders or beneficiaries of the Hospital Victoria Eugenia de Sevilla Card may benefit from the Special Rate.
The Hospital Victoria Eugenia de Sevilla Card is at all times the owner of the card, the holder being only a depositary of it. It is not a means of payment, and is valid at the Hospital Victoria Eugenia Hospital de Sevilla as long as the programme lasts.
The issuance of each card carries a fee.
The card is non-transferable.
The Hospital Victoria Eugenia de Sevilla is to be presented when one visits the hospital, along with the Cardholder’s official identification document (ID card, Driving License or Passport).
Minors must be accompanied by a parent or guardian, who must identify themselves as such.
Loss or damage
The Cardholder must report the card’s loss or damage on site at the Hospital Victoria Eugenia de Sevilla.
The issuance of a new card carries a fee.
Jurisdiction
- Only holders or beneficiaries of the Hospital Victoria Eugenia de Sevilla Card may benefit from the Special Rate.
- As a holder of this Card one may access the services offered in the hospital’s Portfolio of Services at special rates. Only Medical Consults may be accessed directly. For everything else the corresponding prescription of a physician is necessary.
- Cardholders may make appointments for the Hospital’s s Outpatient Care through the appointment programme on the website at www.hospitalveugenia.com
- Cardholders will enjoy early access to information, before anyone else, via email. All information will be sent to the email address provided by the Cardholder. Cardholders who change their email address are to report the new one to the Hospital Victoria Eugenia de Sevilla.
- Cardholders will have access to a Service Centre (telephone service).

In those cases in which your treatment requires the use of prostheses or any other specific surgical material, your personal data may be transmitted to suppliers, solely and exclusively for this purpose.
At the Hospital Victoria Eugenia de Sevilla we aim to offer you service in accordance with your needs and expectations. To this end, we inform you that the Hospital is conducting telephone surveys to measure our patients’ satisfaction. If you do not wish to participate in our service improvement programme through these telephone surveys, please check the appropriate box and your data will not be used for this purpose.
If you wish to abandon this program you may do so at any time by informing our Hospital staff on the Avda. de la Cruz Roja, No. 1 C.P., 41009, Sevilla.
